4-7-8 breathing for high blood pressure: a five minute habit, and its limits
In for four, hold for seven, out for eight. It takes a minute, it costs nothing, and it is the easiest place to start with blood pressure. It is also not the whole answer.
4-7-8 breathing is a five minute relaxation technique: breathe in through the nose for 4 seconds, hold for 7, and breathe out slowly through the mouth for 8, four times over. Slow breathing like this calms the nervous system and can lower blood pressure a little in the moment. It is a useful daily habit for people with high blood pressure, but it works alongside food, movement, sleep and medication, not instead of them.
We teach it in the clinic because stress sits behind so much of what we see: late dinners, broken sleep, the evening biscuit tin, and blood pressure readings that are higher in the doctor’s room than at home. Five minutes of breathing will not undo all of that. It is simply the easiest place to start.
How to do 4-7-8 breathing
The counts matter less than the ratio. If seven seconds is too long at first, shorten every count and keep the out-breath the longest part.
Why slow breathing helps
A long, slow out-breath shifts the body towards its rest and digest state, run by the parasympathetic nervous system. Heart rate eases, and the stress response that tightens blood vessels quietens down. Studies of slow, paced breathing have found modest, short-term drops in blood pressure. The bigger benefit we see in practice is indirect: better sleep, fewer stress-driven snacks, and a calmer start to the day.
Breathing is the habit that makes the other habits easier. It is not the one that replaces them.
What actually moves blood pressure
The changes with the largest, most reliable effect on blood pressure are ordinary ones:
- Salt. The World Health Organization advises under 5 grams of salt a day, about one level teaspoon, and most Indian diets are well above it. Most of it hides in pickles, papad, namkeen, bread, sauces and restaurant food rather than the salt shaker.
- Potassium and fibre. Vegetables, fruit, dal and whole grains, which is what the plate method is built on.

Can 4-7-8 breathing replace blood pressure medication?
No. It can support lower readings and help with stress and sleep, but medication decisions stay with your doctor.
How often should I practise?
Once a day to begin with, then twice: after waking and before sleeping. Four cycles take about a minute, so a round fits anywhere.
Is it normal to feel light-headed?
A little light-headedness in the first few days is common. Shorten the counts and breathe more gently. If it continues, stop and speak to your doctor.
